How do I obtain an absentee ballot for an upcoming election?

Voters wishing to receive an absentee ballot by mail must submit a request in writing to the Auditor's Office. Requests must be received in the Auditor's Office 15 days prior to the election. The request must include:
Name
Date of birth
ID number
Iowa residential address
Address where the ballot is to be mailed if it is to be mailed to an address other than where the voter is registered
Name or date of the election
Party affiliation (Primary Elections only)
Signature and date the form was completed
Mail the signed request to:
